AZZ Card File (here: http://www.azzcardfile.com/) is a great free-form database much like an digital rollodex. You create separate files (databases) and then create cards (records) in them. You can add just about anything to a card including links and photo images.
I have one file for clients that I need to keep handy information about, one for code snippets that I don't use often enough to remember but need frequently enough I don't want to be bothered looking them up in a book, one for passwords, websites, and other online things, and one as an address book with birthdays and other goodies I need to access from time to time. It's a breeze to switch between the files (top menu: file --> open), to select cards (left menu), to add new cards (top menu: card --> new), and to add to a card (just type, copy/paste, or drag/drop).
Keep the shortcut on your desktop or in your quick laumch menu and it's there when you need it.
It's shareware, so it's free to try with a periodic nag screen. It's just $19 to register so support the guy, huh?
